The incident occurred at the Riverbend Maximum Security Institution in Nashville. This failed attempt forced her emergency transfer to a hospital, while specialists debate the causes of this unprecedented event in the country.

Who is Christa Pike and What Happened in the Execution Chamber?

Pike was to be the first woman executed in Tennessee in more than 200 years. She is serving a death sentence for the murder of Colleen Slemmer, a crime committed in Knoxville in 1995. The U.S. Supreme Court revoked a previous stay in the case, allowing the procedure to proceed.

At 7:27 p.m., the execution chamber curtains opened. The inmate received two syringes containing 50 milliliters of pentobarbital each, a fast-acting barbiturate. According to eyewitnesses, Pike questioned the normalcy of the process and warned that her arm appeared ready to burst. After several curtain closures and openings, the defense notified that the woman was snoring loudly and remained alive.

Hypotheses About the Lethal Injection Failure

Currently, there are two main hypotheses about the procedure’s failure, proposed by the legal team and medical specialists.

Intravenous Access Failure

The first hypothesis points to a direct problem with venous access. Joel Zivot, a defense medical expert, told the BBC that the drug did not reach the necessary concentration in the blood. If an IV line is improperly placed, the medication leaks into surrounding tissue instead of entering the vein.

Attorneys had warned in January that Pike has small veins, which complicates needle use. After the failed procedure, the legal team stated that their previous warnings about difficult access and burst veins were confirmed.

Possible Use of Degraded Drugs

The second possibility points to the quality of the drug administered. The defense included the use of «degraded pentobarbital» as one of the possible causes of the failure. To date, authorities have not released technical studies on the batch administered that would confirm this claim.

What Do Authorities Say About the Case?

The Tennessee Department of Correction stated in a release that personnel followed every step of the state protocol. The agency maintained that the lethal injection chemical has proven to be consistently effective.

Additionally, the institution clarified that the current manual does not allow for additional procedures if the person survives the backup dose. However, no official authority explained why the condemned woman did not die.

Robin Maher, director of the Death Penalty Information Center, called the event singular and incomparable. Until this case, no one had survived after receiving the full amount of lethal drugs prescribed for an execution.

Following the incident, Governor Bill Lee ordered an independent investigation to determine whether the failure was in the medication, the venous access, or both factors. Additionally, he suspended the only other execution scheduled for 2026. The condemned woman’s legal and medical future will depend on the results of this review and the condition of her injuries.
