The decline is already being felt in several states and hits harder those who depend on the ACA marketplace to secure medical care. In many households, the problem is not just losing the plan, but being exposed to medical debt that is difficult to manage.

Federal data shows that enrollment dropped by 2.6 million people in February compared to the same month last year. The decline is linked, above all, to the end of reinforced subsidies that reduced premiums for several years.

Why Did Coverage Fall?

The main change was the expiration of expanded federal subsidies. When those tax credits disappear, the monthly payment increases and, in many cases, becomes impossible to pay. That explains why several people simply stopped renewing.

Ohio and Oklahoma recorded the largest proportional declines, with more than 32% fewer members each. Then came Arizona, South Carolina, Minnesota, Indiana, Michigan, Mississippi, Louisiana, and Missouri, all with losses exceeding one-quarter.

KFF Vice President Cynthia Cox explained that the new data count both those who enrolled and those who paid their first premium to maintain insurance. They also include those who were removed for failing to pay on time.

The underlying reading is clear. It’s not just about fewer new enrollments. There were also people who appeared to be covered but fell through due to lack of payment or loss of grace period.

How to Know If It Affects You?

The first step is to check if you received a renewal letter or notice of a premium change. If your payment increased significantly, you likely lost part of the subsidy or no longer qualify on the same terms. That signal is usually the clearest.

You should also verify if your plan is still active in the state marketplace or on Healthcare.gov. In several states, the federal marketplace concentrates the majority of policies, and the impact there has been greater. If your state manages its own exchange, the effect may be different.

If you didn’t pay the premium on time, the plan could have been canceled after the grace period. In some cases, coverage can be terminated retroactively. That means a recent appointment or prescription could be left without protection.
